As of January 2022, the World Health Organization recommends baricitinib for people with severe or critical COVID-19. Thereafter in February 2022, an exploratory randomized, placebo-controlled trial of participants receiving invasive mechanical ventilation or extracorporeal membrane oxygenation were randomly to baricitinib or placebo. There was a 46% statistically significant relative reduction in all-cause mortality for participants receiving baricitinib compared to those receiving placebo at 28 days which was sustained at 60 days. In March 2022, the RECOVERY trial reported that the use of baricitinib cut the risk of death by about a fifth in about 12,000 participants. Then in May 2022, the FDA approved use of baricitinib for the treatment of adults hospitalized with COVID-19 who require supplemental oxygen, non-invasive or invasive mechanical ventilation, or extracorporeal membrane oxygenation with a recommended dose of 4 mg once daily for 14 days or until hospital discharge, whichever happens first.

It is produced in relatively large amounts as a fission product, and decays to xenon-135, which is a nuclear poison with the largest known thermal neutron cross section, which is a cause of multiple complications in the control of nuclear reactors. The process of buildup of xenon-135 from accumulated iodine-135 can temporarily preclude restarting a reactor that has been shut down. This is known as xenon poisoning or "falling into an iodine pit".
== Health and safety regulations == According to Merck Life Science UK 2020 Safety Data Sheet provided to the European Chemicals Agency's Registration, Evaluation, Authorisation and Restriction of Chemicals (REACH)—a 2006 European Union regulation, ethylene oxide is "presumed to have carcinogenic potential for humans." The United States EPA published an advance notice of proposed rulemaking (NPRM) in the 12 December 2019 Federal Register seeking to limit EtO emissions. Over the next couple of years, information was collected and a proposed air toxics rule published in the 13 April 2023 Federal Register. Following a 60 day comment period that could be extended, due to many comments requesting an extension, the EPA rules that could reduce EtO emissions, both direct and fugitive, by over 80% could be implemented within 18 months of publishing the final rule in the Federal Register. Laboratory EtO emitters would still be exempt from the stricter compliance. Additionally, while effectively curbing EtO emissions in the USA, many industrial emitters may simply shift their EtO production to nearby less strict countries: Canada, Mexico, etc. In 2024, U.S. probed claims that popular Indian curry brands MDH and Everest Spices carried ethylene oxide after Hong Kong and Singapore found the contamination in the products and took enforcement actions against them.

== National differences and how to specialise == Australia In Australia, transfusion medicine is a sub-specialty of haematology. Training in transfusion medicine is covered by the Royal College of Pathologists of Australasia (RCPA). Australia has national blood services operated by the Australian Red Cross Blood Service. There are a series of guidelines and standards relevant to the laboratory released by the National Association of Testing Authorities, Australia (NATA), Australian and New Zealand Society of Blood Transfusion (ANZSBT) and RCPA. Similarly, there are a series of clinical practice, patient blood management guidelines by the National Blood Authority. In Australia, the Serious Transfusion Incident Reporting (STIR) system is in place to capture serious transfusion incidents and near-miss incidents.
In a meta-analysis conducted internally by the FDA during its evaluation of esketamine for treatment-resistant depression, the FDA reported a standardized mean difference (SMD) of esketamine for treatment-resistant depression of 0.28 using the three phase III short-term efficacy trials conducted by Janssen. This was similar to an SMD of 0.26 for olanzapine/fluoxetine for treatment-resistant depression and lower than SMDs of 0.35 for aripiprazole and 0.40 for quetiapine as adjuncts for major depressive disorder. These drugs are less expensive than esketamine and may serve as more affordable alternatives to it for depression with similar effectiveness. Both rTMS and intranasal esketamine are more effective than starting a new antidepressant for treatment-resistant depression, with rTMS potentially offering slightly greater or comparable symptom reduction compared to esketamine. Racemic ketamine produces larger and more sustained antidepressant effects than esketamine, with higher doses generally more effective; both ketamine and esketamine have similar dropout rates. Preliminary research suggests that arketamine, the R(−) enantiomer of ketamine, may also have its own independent antidepressant effects and may contribute to the antidepressant efficacy of racemic ketamine, but more research likewise is needed to evaluate this possibility.

Therapeutic drug monitoring (TDM) is a branch of clinical chemistry and clinical pharmacology that specializes in the measurement of medication levels in blood. Its main focus is on drugs with a narrow therapeutic range, i.e. drugs that can easily be under- or overdosed. TDM aimed at improving patient care by individually adjusting the dose of drugs for which clinical experience or clinical trials have shown it improved outcome in the general or special populations. It can be based on an a priori pharmacogenetic, demographic and clinical information, and/or on the a posteriori measurement of blood concentrations of drugs (pharmacokinetic monitoring) or biological surrogate or end-point markers of effect (pharmacodynamic monitoring). There are numerous variables that influence the interpretation of drug concentration data: time, route and dose of drug given, time of blood sampling, handling and storage conditions, precision and accuracy of the analytical method, validity of pharmacokinetic models and assumptions, co-medications and, last but not least, clinical status of the patient (i.e. disease, renal/hepatic status, biologic tolerance to drug therapy, etc.). The experimental determination of pKa values is commonly performed by means of titrations, in a medium of high ionic strength and at constant temperature. A typical procedure would be as follows. A solution of the compound in the medium is acidified with a strong acid to the point where the compound is fully protonated. The solution is then titrated with a strong base until all the protons have been removed. At each point in the titration pH is measured using a glass electrode and a pH meter. The equilibrium constants are found by fitting calculated pH values to the observed values, using the method of least squares. The total volume of added strong base should be small compared to the initial volume of titrand solution in order to keep the ionic strength nearly constant. This will ensure that pKa remains invariant during the titration. A calculated titration curve for oxalic acid is shown at the right. Oxalic acid has pKa values of 1.27 and 4.27. Therefore, the buffer regions will be centered at about pH 1.3 and pH 4.3. The buffer regions carry the information necessary to get the pKa values as the concentrations of acid and conjugate base change along a buffer region. Between the two buffer regions there is an end-point, or equivalence point, at about pH 3. This end-point is not sharp and is typical of a diprotic acid whose buffer regions overlap by a small amount: pKa2 − pKa1 is about three in this example. (If the difference in pK values were about two or less, the end-point would not be noticeable.) The second end-point begins at about pH 6.3 and is sharp.
== Prognosis == The use of DMARDs in the management of JIA have made disease-free remission achievable for the majority of children with JIA. Clinical remission can be defined as the absence of signs and symptoms of inflammatory disease activity, including extra-articular manifestations of the disease. Differentiating subtypes of JIA helps to target treatment and leads to more positive outcomes, however subtype is not the only predictor of JIA outcome. Poor prognostic factors include arthritis of the hip, cervical spine, ankles or wrists; prolonged elevation of inflammatory markers; and radiographic evidence of joint damage including erosions or joint space narrowing. Patients with RF-positive polyarthritis often have worse outcomes associated with more aggressive disease. Despite this, the probability of this subgroup achieving inactive disease at least once within five years was shown to be 90% in a large Canadian study. Research is currently being undertaken into clinical prediction models to allow earlier identification of children who are likely to have a worse prognosis. Compliance with therapy, especially medications, has a positive correlation with disease outcome. Disease recurrence after medical therapy discontinuation is high, with 30-100% of children having clinical relapse of disease after treatment discontinuation.
